This generates a message that is sent to Argyll and Bute Health and Social Care Partnership (HSCP) Public Health team. By submitting this form, you are providing consent for us to use the information you provide as described below.

Depending on what you have been in contact about, you will receive a reply from either a member of the Argyll and Bute HSCP Public Health or your details will be shared with your local Living Well Network and they will reply to you.

 Living Well Network (LWN) membership – Privacy Note

Why do we ask for your contact details?

We value your interest and involvement with your local LWN. The LWNs were set up by Argyll and Bute Health and Social Care (HSCP) Public Health team. The LWNs operate to bring together people with an interest in health and wellbeing in their local area and allow sharing of information. The LWNs inform their members via emails from the LWN coordinators. We hope that the information, events and networking are useful to you in your profession and community role. The lawful basis for processing your information is that of ‘Public Task’.

What will we do with this information?

LWN members may be emailed by the local LWN coordinator about matters related to health and wellbeing including:

·         Network bulletins relating to health and wellbeing

·         Upcoming events including training events and workshops

·         News

·         Local LWN meetings

·         Surveys on behalf of the LWN, Argyll and Bute HSCP or partners

·         Information from Argyll and Bute HSCP or Public Health team

 Your details will be shared with Argyll and Bute HSCP Public Health team. You may also receive emails from Argyll and Bute HSCP Public Health team on any of the above relating to health and wellbeing.

 How will we keep your information

Details held will be:

·         Members name

·         Organisation worked for (if applicable)

·         Position or job title (if applicable)

·         Contact email address

·         Contact phone number

Your details will be stored by your local LWN coordinator and by Argyll and Bute HSCP Public Health team.

If a LWN coordinator resigns from their role, they will delete their copy of the distribution list. Your contact details will not be shared with any other parties. Argyll and Bute HSCP Public Health Team hold data on a secure NHS Server.

Your rights

You have the right to leave LWN and this can be done by emailing your network coordinator. Your details will then be deleted. Your contact details will also be removed if your email address rejects a message that has been sent.

 Living Well (Self Management) Grants – Privacy Policy - 30 Nov 2021

All applications

We need to process your information under the lawful bases of ‘contract’ and to fulfil our ‘public task’.

Your application form will be shared with the Argyll and Bute Health and Social Care Partnership (HSCP) Public Health team for the purposes of recording and auditing the process of awarding grants. It will be stored on a secure NHS drive which only members of the Argyll and Bute HSCP Public Health team have access to.

Your application form will be shared with a scoring panel to enable them to score your application. The scoring panel will include: members of Argyll and Bute HSCP, Living Well steering group members, and a representative from the Third Sector Interface

.Contact details in applications

Your contact details from your application may be used to get in touch with you:

·         if the scoring panel need further information in your application

·         for feedback on, and evaluation of, the effectiveness of the Self Management grant

·         in reference to your invoice, if you are successful

·         about your monitoring report, if you are successful

·         for other reasons related to the Self Management grant

Unsuccessful applications only

Unsuccessful applications will be kept by the Argyll & Bute HSCP Public Health team and you may be contacted by them in the future regarding your application. We do this to help support potential projects. Your application will not be kept for longer than one year after the end of the financial year that the application was made in.

Successful applications only

The details from your application including your name, organisation and contact details will be added to a Self management grant database. The database is stored on an NHS server and is maintained by Argyll and Bute HSCP Public Health team.

Details of successful projects will also be shared in the Public Health team annual report and be widely shared through social media and at relevant meetings such as the Community Planning Partnership groups and relevant HSCP meetings and reports.

Use of the Self Management grant database

The details from your application that are stored on the database will be used for a number of things:

·         To email you your notice of success

·         To send you your monitoring report form, when it is due

·         To record whether monitoring information has been completed

·         To monitor the distribution of the SM grant

·         To evaluate the SM grant effectiveness

Grant application invoices

Grant application invoices will be seen by Argyll and Bute HSCP Public Health team and NHS Highland finance department. The finance department may get in touch with the contact details on your invoice if they have any queries regarding your invoice.

Monitoring reports

You are asked to provide case studies to support the evaluation of your project. These will be published on the Living Well (previously Healthy Argyll and Bute) Website – www.ablivingwell.org

It is your responsibility to ensure that the information you provide does not contain personal information and that there is no copyright for the information provided.

Duration of application and supporting documents storage

Your application and relevant supporting documents to your grant, i.e. invoice, monitoring report, and the details added to the database will be stored by the NHS in line with the Scottish Government guidance on Records Management for the NHS. This is to ensure monitoring of all funds granted can be evidenced and reported on.

Your rights

You have the right to request to see what personal information we hold and for the information held to be corrected if appropriate. Please do inform us of any changes to the contact details that you have provided us.

 Argyll and Bute Health and Social Care Partnership Public Health team Privacy policy – Training and Events (Nov 2021)

 Why do we need this information?

We record this information to monitor who has booked on and attended the training and events we offer and to be accountable for the funding provided for these events. Your information is therefore held due for us to carry out our ‘public task’.

We may also ask about dietary and access requirements for the training or event but this information will only be used for planning the event and not stored further.

 We will use your contact details to provide you with any information about this event e.g. location, time, programme, any preparation needed. We will also contact you regarding evaluation of the event that you have attended.

 In some circumstances we may ask for your consent to share your details with other NHS bodies such as NHS Education for Scotland, or other training providers.  If the training provider is not an NHS body, we will also direct you to their privacy policy.  In these cases we will tell you which information we will share, and explicitly ask for your consent for this.

 We would also like to be able to inform you about future events or projects including development days and training events, related to health and wellbeing in Argyll and Bute. We will only contact you about other events if you consent to this.

 How long will we keep your information?

You details will be stored on secure NHS servers. We will not share your details with anyone outside of Argyll and Bute HSCP Public Health team, except with your permission in the instances detailed above.

 We will keep a record of your attendance at our events for up to 7 years after the event. After this, the record will be deleted.

 If you ask to be contacted about future events, you will be told how to be removed from this distribution list each time you are emailed.

 Your rights

You have the right to change your mind about consenting to being contacted about future events or projects. Please contact us by email to inform us of this. You have the right to request to see what personal information we hold and to have information updated and rectified if it changes.
